A Custom Action that creates a new project from an Enhancement Request and links the Request to the new project.

A Custom Action that creates a new project from an Enhancement Request and links the request to the new project.
The user provides:
- name (automatically set to request name)
- template (optional)
- manager
- start date
- portfolio (optional)
Upon creation of the project, the request is linked to the new project.
